Our hospice care services main goal is to help ensure that your loved one has an enriched end-of-life experience. We create a customized plan of care for each of our Fremont hospice patients, taking a holistic approach to address their individual needs. Our hospice care team is composed of doctors, registered nurses, social workers, bereavement specialists, therapists, chaplains and skilled volunteers, all of whom are specially trained in caring for those who are approaching the end of life.
